My Place CT My Place CT provides a one-stop resource for the information and connections people in CT need. How can we help? My Place CT is part of Governor Dannel P.

MyPlaceCT is all about helping older adults and people with disabilities connect to resources to live a fuller life at home. The MyPlaceCT website is the only comprehensive, one-stop resource that explains the many ways to make that happen. My Place CT is the state’s single source of independent, well-vetted information with no commercial agenda. You’ll find information, providers of services and in-person help to live at home or in the community. My Place CT is also for families and caregivers looking for support for their loved ones. We all want to live where and how we choose. But it isn’t always easy to find what you need to live independently at home. My Place CT supports a growing network of community partners to help you find the help you need. The goal is to have experts in every community who can navigate the system and help individuals find local services and support. Care Through Community partners can help with:
• Determining needs - Help assessing what you need
• Finding care and support – help finding and hiring programs and services
• Paying for care – accessing state benefits and ideas for affording help
• Housing and transportation – options and resources

How to find us
Look for the purple How can we help? sign at senior centers, libraries and other locations in your community. If your town doesn’t have a network yet, you can call 211 for help finding what you need. Or, go to the Finding Care and Support section of MyPlaceCT.org. My Place CT is sponsored by the Connecticut Department of Social Services, in collaboration with 2-1-1/United Way of Connecticut and community partners statewide. Malloy’s Strategic Plan to Rebalance Long-Term Services and Supports, promoting independent living choices for state residents and further developing home and community living options. Funding is provided by the federal Centers for Medicare and Medicaid Services.

If you or a loved one has been in a long-term care facility for 60 days or more, Money Follows the Person (MFP) can help you get back home or back to the community. How can we help? MyPlaceCT.org is a free, web-based resource designed to help older adults and people with disabilities live at home or in the community with optimal health and well-being. It helps people explore their options and features current information on a wide range of services and supports - all in one place.

As a virtual home of No Wrong Door (NWD), MyPlaceCT.org reflects the philosophy that each person is unique, with individual needs, on their own journey to seek long term services and supports. Regardless of where an individual starts, partners and providers can help people utilize MyPlaceCT.org to better understand and access their options and support their decision-making.

The easily accessible information and the opportunities for training featured on MyPlaceCT.org, will help a variety of stakeholders - providers, communities, Partners - maximize time, create efficiency and have an even greater impact on the people they serve.
