I have not been very active on Instagram the past year since we moved to Vegas, but couldn’t resist posting this recall I got from one of my co-residents.

18 month recall on my first intentional replantation case. Asymptomatic, functional and healing!

Non-dental friends, graphic pictures in this post so keep scrolling if you don’t want to see!

Pt saw a provider at a different clinic a week before, with the note stating specifically that “no perforations were noted in access prior to closing” 🤷🏻‍♂️. The PAs obviously told a different story.

Pt was aware of guarded prognosis but still wanted to save the tooth.

Upon access, not 1 but 2 perforations noted. Canal located and instrumented to 35/04 with edge X7.

Obturated with BC hi flo and modified warm vertical.

Perforations were scrubbed with cotton pellet soaked with NaOCl, and BC putty was packed into perfs.

Access sealed with BC liner.

Fingers crossed for the 3 month follow up!

Tip: when there is a perf present, try to isolate the perforation to minimize NaOCl leakage into the surrounding tissues. I packed cavit into the perf area prior to irrigating.

I was just talking to my co-residents at lunch today about how I haven’t really gotten many lower molars with multiple distal canals. I guess I jinxed myself! 6 canals on this lower molar, the most canals I’ve had on a tooth so far!! MB, MM, ML, DB, Mid distal?, DL. Pt did not want a cbct. Next time I’m going to insist on it 😆. Edge X7 with BC hi flow. Permanent core placed.

Protaper ultimate and gentlewave!
Got to try out protaper ultimate. Really like the system and the conservative shapes we can get with it! Loved using the slider (16/02) for glide path. Only hand filing done here was with a 15 to determine working length. Finished all canals to a F1 (20/08), gentlewave irrigation, obturated with matching cones and BC hi flow.

Earlier this week we got to try out a guided endodontic microsurgery using the GEMS protocol. I brought the patient in for an intraoral scan, exported an stl file and sent that with the DICOM of the CBCT to have a surgical stent created. After elevating a FTMP flap, we then used a trephine to remove the root end and cortical bone above the root. Retroprepped 3mm and sealed with BC putty. No bone graft needed. Awaiting pathology results from lesion, but is likely a cyst. Loving the opportunity to try new technology and techniques during residency!!

Another gentlewave case. #5 PT/AAP. Some wierd paste in the L canal and very loose and radiolucent gutta percha in the buccal. Either way pretty straightforward case. No crazy anatomy or laterals captured by the GW and not even a very difficult case but very large lesion on both the palatal and buccal canals. 25.04 X7 used with BC Hi Flo. BC liner placed as final build-up. Excited to see how the 3 month recall turns out!!
