Total Knee Revision and Extensor Mechanism reconstruction with full extensor allograft(tibial tubercle/patella tendon/patella/quad tendon). Thanks for teaching me how to do these difficult cases. Patient had previous TKA with post op patella fracture treated with attempt at primary repair then patellectomy and Achilles allograft with repeated extensor mechanism failure before referral. Decided to revise components to better balance the knee and change component positions.

Family support…being a doctor takes a commitment and is stressful and the lifestyle can be hard on families with late nights and early mornings but as I sit here doing charts and admin work having these reminders of my family brings me joy and support.

Even some days you walk in the room and are surprised. Patient had chronic poorly controlled gouty arthritis with large tophi(deposits of gout/Uric acid). Able to excise the tophi and do a total knee and looks like a completely different knee.

Difficult case… well ingrown stem that had to be removed for infection. Previous anterior approach with anterior wound. Tried to debride wound and remove through anterior approach had to be converted to posterolateral approach and use an extended trochanteric osteotomy. Stem was completely ingrown the full length of the stem. Used flexible osteotomes, K wires, pencil tip burr, and slap hammer without success before osteotomy. Interested if anyone would have done anything else before osteotomy?

Starting off 2022 with a revision hip. Metal on metal spiked cup that had aseptic loosening. In the images you can see previous position before migration then the planned template for revising with TM augment to bring back down the hip center and restore leg length followed by post op with heterotopic bone resection and augmented acetabular revision with bone grafting. .
