Teresa Schwartz, "Teresa was helping Pharmacy with inventory and caught that an extended release medication was being sorted with the regular strength medication. Due to her finding, the medication was moved into the correct bin for correct dispensing."
Trisha Hart, "Trisha (PCT) recognized signs and symptoms of hypercapnic respiratory failure and notified the Respiratory Therapist (RT) on duty that the patient urgently needed to be seen. RT immediately went to see the patient and found her to be very hard to rouse with low oxygen saturations (80%). Blood gases were drawn and were very critical. The patient was immediately placed on Bipap, requiring high levels of respiratory support and was shipped to another facility to receive a higher level of care. Because of Trisha’s knowledge, compassion and quick action, a much worse situation, which was rapidly developing, was avoided which mosly likely would have resulted in emergent intubation, resusitation, or potentially death."
Jessica Haug, PA-C, "Jessica caught an error in the Findings of an Image report (Radiology). Jessica called the Radiologist and spoke with him in regards to his mistake. With further investigation, the Radiologist made the addendum to his report. This was all conducted within 30 minutes and the patient was able to get their appropriate plan of care!"
