Your horse's ear may be very sensitive, this acupressure ear chart is a good description of possible reasons. The equine ears are full of acupressure points that connect to many joints in their body. Sensitivity in the ears maybe a sign of discomfort in other areas of his/her body.

When I got him as a six year old, the veterinarian doing his dental examination thought he had possibly had a broken jaw. Turns out that he was a teeth grinder and had a parrot mouth. Poor guy. His teeth are fine now but he still gets tightness in his jaw and needs his TMJ and Mandible released.

Danny demonstrating a release. You can't see it in the video but he also raised and stretched his right hind after he finished pawing. Notice how he is aslo lowering his head and stretching his neck both ways. If there are blocked or problematic areas in the thoracic sling they can effect other areas throughout horse's body.
