Patient presents to the office with a complaint of pain in the great toenail. Dried blood was noted, and the toenail was partially loosened. The affected toenail was debrided, and postoperative wound care instructions were reviewed. The patient was advised that a new toenail is expected to regrow in approximately 9–12 months.

Damaged Toenail

A damaged toenail bed—often caused by trauma such as stubbing, crushing, or dropping a heavy object on the toe—can lead to pain, swelling, discoloration, and sometimes blood trapped under the nail (subungual hematoma). In more severe cases, it may result in nail loss, infection, or permanent nail deformity.

⚠️ Toenail injuries involving the nail bed can be a medical emergency.

Dangers of Smoking

A patient with a 50-year history of smoking now presents with a painful dry gangrene of the toe due to severe circulation problems.

The patient has undergone a vascular procedure with placement of a stent in the leg to improve blood flow. Once the level of healing is fully determined, the patient has elected to proceed with surgical amputation.

Smoking significantly increases the risk of poor circulation, delayed healing, infections, and limb loss.
