Reverse T3: The Reason You’re Exhausted Even When Your Thyroid Labs Are “Normal”
One of the most frustrating things I see in practice is patients who feel completely exhausted, struggle with weight, and just do not feel like themselves, yet they have been told their thyroid labs are normal.
What many people do not realize is that there is another thyroid marker called Reverse T3 (RT3) that can play a significant role in how you feel, even when your standard labs look fine.
RT3 is essentially a survival response. It is what your body produces when it senses stress and needs to conserve energy rather than create it. This can be triggered by chronic stress, inflammation, calorie restriction, illness, overtraining, or even certain medications. When RT3 levels rise, it can block your active thyroid hormone from doing its job, leaving you feeling sluggish, tired, and stuck, despite being told everything is within range.
That is why looking at a single number is rarely enough. A complete thyroid evaluation gives us a much clearer picture of what is really happening in your body and helps us understand why you may still be struggling.
When we evaluate thyroid function comprehensively, we typically look at:
• TSH
• Free T3 and Free T4
• Reverse T3 (RT3)
• Thyroid antibodies
Because being in range does not always mean you are functioning optimally, and it certainly does not mean your symptoms should be ignored.
