A new equation that includes resting HR better matches true HRmax and may help create more achievable, individualized exercise targets for this population.

Neuro Nugget 🧠: Loaded Dix-Hallpike improves BPPV detection

A randomized trial compared the standard Dix-Hallpike to a modified “loaded” version for posterior canal BPPV. Pre-loading the head in rotation and flexion before lying supine produced longer nystagmus responses, stronger symptoms, and higher diagnostic sensitivity than the standard test.

🔑 Key takeaway:
Using a Loaded Dix-Hallpike can improve BPPV detection by increasing sensitivity and nystagmus duration, helping guide more effective treatment.

Cooling garments aren’t just about comfort in MS.

A recent systematic review and meta-analysis found that wearable cooling (vests, limb cuffs, palm cooling) is associated with improved walking capacity and functional mobility—with some studies also showing gains in strength and balance.

For neuro PTs, this matters.
Cooling may help reduce heat-related symptom worsening, allowing patients to tolerate more gait, endurance, and strengthening work during rehab.

Sometimes the environment is the intervention.

No ankles ≠ no gait training.

Owen is an incomplete paraplegic who ambulates with bilateral AFOs and has minimal distal strength, limited glute max, and little to no glute med activation. Instead of forcing compensation in standing, we challenge him where it matters most 👇

Why tall kneeling works:
✔️ Removes ankle and foot strategy
✔️ Demands true proximal stability
✔️ Loads hip extension + trunk control
✔️ Exposes asymmetries you can’t hide from

Forward and backward tall kneeling walking builds strength, control, and motor carryover that shows up when he returns to upright gait.

Train the system in different ways - variability matters.

Leg management after SCI is not one-size-fits-all.

For individuals with cervical spinal cord injury, how we manage the legs depends on so many factors — height, weight, arm length, trunk control, strength, and even momentum strategies.

In this session, my client with a C6–7 spinal cord injury demonstrates excellent leg management by starting proximal on the leg, using momentum to bring the leg onto the mat, and then applying a strong head–hips relationship to get the pelvis over and centered.

💡 Neuro Nugget:
Start proximal for control → finish distal for the final push.
And remember: good technique isn’t just strength — it’s timing, momentum, and mechanics.

This is how we build efficient, repeatable, and independent mobility after SCI.

Why does knee hyperextension happen in gait? 🤔🦵
Let’s break it down.

In neuro rehab, knee hyperextension is rarely caused by just one factor — it’s a combination of strength deficits, motor control challenges, and sensory impairments working together.

Here are the most common contributors:
🟦 Quad weakness
🟦 Hip extensor weakness

🟦 Plantarflexor tightness / DF weakness

Calling all Neuro PTs! 👋💙

In today’s session, our client is working on step-ups with XCITE targeting the tibialis anterior to improve toe clearance and safe foot placement during functional mobility. 🔌🦵

However, you’ll notice she demonstrates knee hyperextension, which is common secondary to weakness, proprioceptive deficits, and impaired motor control. While today’s focus was strictly on improving dorsiflexion activation, it opened the door for great discussion around how we’ll address hyperextension in future sessions — including:

✨ Targeted XCITE activation for quads, hamstrings, and gastroc
✨ Proprioceptive training and reactive balance work
✨ Visual feedback with mirrors to improve awareness
✨ Strengthening and motor control strategies to promote safe knee mechanics
