Dear valued patient,

We wanted to let you know that MediCenter – Kachemak Bay Medical Clinic will be closing effective 11/04/2019. Due to this change, all providers will no longer be available to meet with patients after 10/30/2019. If you are due for a prescription refill for the month of November, please call 907-235-7000. There may be some technical issues so you may have to call more than once.

Dr. Paul Raymond will be starting a new primary care practice called Kachemak Medical Group which will be opening at the beginning of November.

Medical Records from MediCenter & Kachemak Bay Medical Records will be available for transfer. Please fax requests to 844-237-3959 or email request to medicalrecords@kenaidoctor.com.

MediCenter wants to make sure that patients have continuity of care, patients can contact Kachemak Medical Group for their care.
