We’re excited to celebrate our therapy teams in New Jersey, Virginia, and Connecticut for their impressive Quality Measure (QM) results. These communities earned #1 rankings in their city or area, or placed in the Top 10% statewide for Short-Stay Discharge Function Score.

This measure highlights the meaningful functional progress residents make during their stay — progress driven by expert care, strong teamwork, and a shared commitment to excellence.

Since the start of Together Making Changes, our goal was bold: $2 million in total giving by the end of 2025.

💥 We needed $181K in 2025 — and hit it by JUNE.

🎉 Then we went even bigger, making 2025 our largest year ever with $320,190.42 raised for Together Making Changes.

✨ December alone brought in $32,467.43.

We’re proud to recognize our Mississippi therapy teams who achieved outstanding Quality Measure (QM) rankings — earning #1 in their city or area, or placing in the Top 10% statewide for Short-Stay Discharge Function Score.

This quality measure reflects how much residents improve functionally from admission to discharge — a direct result of skilled therapy, collaboration, and consistent, resident-centered care.
