A recent Double Take video presents the case of a 42-year-old woman presenting with cough, fatigue, fevers, and shortness of breath. Based on a Clinical Problem-Solving article published in NEJM, the video explores the differential diagnosis for shortness of breath and highlights the importance of maintaining a broad differential when first-line therapies fail to improve a patient’s condition. https://nej.md/4g7I8hq

I hate to post a negative post at such a wonderful time of the year, but for physicians, there’s definitely a feeling that the Grinch had a hand in the final version of the bill passed by Congress this weekend.

At the last minute, the decision was made to exclude important healthcare related provisions in the initial versions of the bill that were key to protecting access to healthcare.

The decision was made not to address the 2.83% Medicare paycut that physicians will get starting Jan 1. Given that physician reimbursement makes up only a tiny % of expenditure, these cuts will do a lot more harm than good.

In the face of a large physician shortage, this will further restrict access to care for as physician practices struggle to address increasing overhead. According to the AMA, when adjusting for the costs of running a practice, physicians have had a 33% cut in Medicare pay rates over the past 2 decades. There have been no adjustments to physician compensation to account for inflation.

As commercial payors follow Medicare rates as benchmarks, this will likely result in insurance companies getting more profits while more physicians opt to stop taking both Medicare and commercial health insurance.

Additionally, this is the straw that breaks the camel’s back for many physicians about the direction healthcare is going in. There is increasing chatter of unionization, strikes, and walkouts.
At a time of alarming burnout, this decision also amplifies thoughts that many physicians have about leaving medicine. With 20-25% of physicians already thinking of quitting within the next 2 years, this is a real public health threat.

Adding insult to injury, provisions that would’ve tightened federal oversight of pharmacy benefit managers were also removed.

A short term 3 month extension of telehealth and acute hospital-care-at-home waivers was passed, although the originally planned 2 year extension would have bought more security for the future of telehealth.

Hoping that Congress works quickly to retroactively address all of these things in 2025. Physicians are tired and are increasingly losing faith in the system.
