Grand County Rural Health Network

We offer healthcare access, resources & support for our neighbors to lead their healthiest lives.

The Grand County Rural Health Network is a nonprofit organization that services both Grand and Jackson counties in Colorado. We believe everyone has the right to health care and to understand how to access that care. We understand that good health encompasses more than health care, and that we are all healthier together. Our dedicated staff works in partnership with health providers, businesses, and community members to improve the future of health systems in our community. We work to educate our neighbors and help them navigate those systems to ensure they have the information and services they need to live their healthiest lives.

Meet our Youth Wraparound Care Coordination Team! Katie Medina joined our team in September 2025 as our Youth Wraparound Care Coordinator. Her previous role as a Family Centered Treatment Trainer and Specialist laid the foundation for her extensive knowledge in working with families to provide hope and partnership in healing and strengthening families. She has a passion for changing the lives of others and planting seeds for ongoing change and growth within families for generations to come.

The Youth Wraparound program supports youth 0-21 who experience complex behavioral health challenges. The Youth Wraparound Coordinator follows a structured, evidence-based process called High Fidelity Wraparound. This process helps the youth and their families create a vision for their future. They follow a plan created by the youth and family and build on the family's strengths and natural support systems.

The program is guided by our Care Coordination Manager, Jeanette Causey. Jeanette has prior experience as a High Fidelity Wraparound Facilitator and Wraparound Supervisor in Routt County prior to re-joining the Grand County Rural Health Network in November 2024.

Do you enjoy talking to people? Are you detail-oriented? Are you good at exploring information and educating people to help them make their own decisions? We are accepting applications for a part-time, seasonal Health Coverage Guide position for November 2025-February 2026.

Become a Certified Health Coverage Guide, providing objective outreach, education and enrollment assistance to individuals and families interested in purchasing health coverage through Connect for Health Colorado, the online health insurance marketplace.

Position is based in Hot Sulphur Springs or Granby and requires daily travel throughout Grand County and monthly travel to Jackson County. Training provided. Bilingual Spanish speakers strongly encouraged to apply.
