The Houma office serves a 50 mile radius including the parishes of Ascension, Assumption, Jefferson, Lafourche, St. Martin, St. Mary, and Terrebonne. Journey Hospice benefits everyone, and is merely a shift in medical priorities, not abandonment. Our number one goal is always patient comfort and pain management delivered at home by professionals. This process eases the financial and emotional burdens on the family, while ensuring the patient's dignity and wishes are respected. Journey Hospice is recognized by all relevant government agencies. We are members of the National Hospice and Palliative Care Organization, and comply with all NHPCO principles. Our Medical Director heads up our Journey Hospice Care Team including the patient's Primary Care Physician, Certified Nursing Assistants, Chaplains, Registered Nurses, Social Workers, Bereavement Coordinators, and Volunteers.

We are looking for a full time LPN that is compassionate and works well with others.

Duties will include:

Provide compassionate nursing care and education to our patients and families under the supervision of the assigned RN Case Managers
Complete appropriate documentation in timely manner
Assist with monitoring and ordering patient care supplies
Inform the physician of changes in the patient's condition and needs, after consulting with a Registered Nurse
Communicate with medical professionals, hospice team and related staff
Report necessary matters to the attention of the Clinical Manager on duty
Participate in Team meeting
Qualifications:

*Current LPN license in State of Louisiana

*Must have current CPR certification

*COVID-19 Vaccine required

*Valid STATE driver's license and current auto liability insurance

Job Type: Full-time

Salary: $22.00 to $25.00 /hour

Summary

The RN Case Manager is an integral part of the hospice interdisciplinary care team and as such is responsible for coordinating the total care of the hospice patient and family. Understanding that hospice care must be delivered according to accepted standards of practice guided by the mission of the organization and the patient/family's needs and concerns, the Case Manager blends all services of the hospice into an integrated program of support.

Duties and Responsibilities

*Provides comprehensive nursing assessments as directed by the interdisciplinary team

*Participates in developing the interdisciplinary team plan of care

*Supervises Licensed Practical Nurses and Certified Nursing Assistants

*Teaches appropriate palliative interventions to family members, other health professionals and the community at large

*Contributes to program development through building relationship with potential referral sources in the area assigned

Qualifications:

*Current registered nurse Louisiana

*Experience with terminally ill patients is preferred

*Must have current CPR certification

*Valid STATE driver's license and current auto liability insurance
