Not all headaches are the same, and proper diagnosis matters.

According to the American Academy of Family Physicians (AAFP), most headaches seen in primary care fall into specific categories. Recognizing the differences helps guide treatment and determine when urgent care is needed.

Here are 6 clinically important types:

🧠 1️⃣ Migraine
One of the most common and disabling primary headaches.
• Throbbing or pulsating pain
• Often one-sided
• Moderate to severe
• Nausea
• Light and sound sensitivity
• Lasts 4–72 hours
Migraines are neurologic — not simply “stress.”

💆 2️⃣ Tension-Type Headache
The most common overall.
• Dull, pressure-like pain
• “Tight band” sensation
• Both sides
• Mild to moderate
• No nausea
Usually not dangerous but can affect daily life.

🔥 3️⃣ Cluster Headache
Less common but extremely severe.
• Intense pain around one eye
• Lasts 15–180 minutes
• Occurs in cyclical “clusters”
• Eye tearing or nasal congestion

💊 4️⃣ Medication Overuse Headache
Occurs when pain relievers are used ≥10–15 days per month, leading to more frequent or chronic headaches.
Often requires medication adjustment.

🌡 5️⃣ Hypertension Headache
The American Heart Association notes mild hypertension usually does NOT cause headaches.
However, severe elevations (≥180/120 with symptoms) may cause severe headache, vision changes, chest pain, or neurologic symptoms. This may represent a hypertensive emergency.

🚨 6️⃣ Thunderclap Headache
Sudden, severe headache reaching maximum intensity within seconds.
May signal hemorrhage, stroke, or vascular emergency. Seek immediate care.

🧠 What Is a Headache?

A headache is pain or discomfort in the head or upper neck. It is a symptom, not a diagnosis.

The brain tissue itself does not feel pain. Headaches occur when pain-sensitive structures around the brain become irritated or activated. These include blood vessels, nerves (especially the trigeminal nerve), the protective layers around the brain (meninges), and the muscles of the scalp and neck.

When these structures become stretched, inflamed, tightened, or chemically stimulated, they send pain signals to the brain, and that is experienced as a headache.

🔎 What Causes a Headache?

Common causes include:

• Muscle tension in the neck and scalp
• Changes in blood vessel activity
• Nerve activation
• Inflammation
• Dehydration
• Illness or infection
• Stress
• Hormonal changes
• Medication overuse
• Severe elevation in blood pressure

The underlying cause depends on the pattern, severity, and associated symptoms.

🩺 Primary vs. Secondary Causes

In primary care, headaches are grouped into two categories:

Primary headaches: the headache itself is the condition. There is no underlying structural disease. These are the most common.

Secondary headaches: the headache is caused by another medical problem, such as infection, head injury, vascular disorders, medication overuse, or severely elevated blood pressure.

Distinguishing between these categories helps determine whether imaging, preventive treatment, or urgent care is needed.
