PCOS is classified as a reproductive disorder, but that’s only because that’s where the symptoms show up first.

But the root cause is metabolic. It’s insulin resistance.

Insulin is what drives the high androgens.

Insulin is what disrupts ovulation.

Insulin is what contributes to weight gain, fatigue, acne, inflammation, and cravings.
PCOS doesn’t start in the ovaries. It starts in your metabolism.

If we keep treating it like a reproductive problem, we’ll keep missing the opportunity to truly help patients.

Elevated insulin isn’t part of the diagnosis.
It’s not part of the treatment.
It’s barely even discussed.

Instead, we treat the symptoms of high insulin levels:

• Irregular periods
• Acne
• Hair loss on the scalp
• Unwanted hair growth

🚫Insulin resistance is not caused by eating fat.
🚫It's not caused by obesity (cart before the horse...)
🚫It's definitely NOT diagnosed with an A1c.

It’s driven by recurrent insulin spikes via overconsumption of starch (yes, even whole grains), sugar, and unfermented dairy.

Even worse when these foods are combined with fat, especially processed fats.

This matters for women with PCOS, where insulin resistance affects up to 95%, yet remains excluded from diagnostic and treatment guidelines.

It's time to start understanding the metabolic pathways at play.

That’s still the standard advice we’re giving women with PCOS.

Meanwhile, up to 95% of them have elevated insulin, a hormone that drives hunger, fatigue, and weight gain.

And then we wonder why they “aren’t compliant.”
Why they “aren’t losing weight.”

Are we explaining how high insulin levels affect their ability to reach their goals? Are we even testing it?

No.

👉Imagine not testing cholesterol in a patient with heart disease.
👉Or not testing glucose in someone with diabetes.
👉Or not testing TSH or free T3 in someone with a thyroid condition.

So why are we ignoring insulin in a condition literally defined by hyperinsulinemia? It's unacceptable. It’s not just a missed lab. It’s a missed opportunity to actually help. To diagnose earlier.
