A 52-year-old man with a history of splenectomy and hypogammaglobulinemia after CAR T-cell therapy for lymphoma 3 years earlier was admitted with septic shock. Encapsulated diplococci were seen in 20% of neutrophils.

It’s been a hectic time with nonstop clinical work and pending projects/manuscripts, so I almost forgot about my feature in this magazine. I’ve shared my personal perspective on the essential components we all need as pathologists. Take a look! 😆🔬

This skin punch biopsy shows spongiotic and superficial perivascular dermatitis with occasional parakeratotic mounds and extravasated red blood cells.

In the appropriate clinical context, these histopathologic features are consistent with pityriasis rosea (PR). However, keep in mind that PR-like drug eruptions can also show similar findings. 😆🔬

It’s been a while since I shared a case.

Did you know this immunoprofile, especially the diffuse TRPS1 immunoreactivity, strongly favors primary EMPD over secondary EMPD? Give it a try if you haven’t used it in your practice. 😆🔬

Oscar goes to Dr. Glaucomflecken. No movie script beats lines like:

“It’s a frozen. It’s not anesthesia trying to wake a patient up.”

“You’re wound up tighter than a keratin pearl.”

I see my surgical colleagues at frozen all the time — hoping one day I’ll catch my gas colleagues there too (after surgery, of course)! 😂🔬

🔥 Hot off the press: I’m tired of people equating the presence of a fusion with a Spitz tumor. It’s NOT that simple—tumors don’t always follow the textbook! Many bona fide melanomas of non-Spitz lineage can harbor fusions.

For example, some BRAF-fused melanomas of non-Spitz lineage may be characterized by prominent organoid and nested growth patterns, rhabdoid cytomorphology, and occasional Reed–Sternberg–like cells (arrow).

🔥 Hot off the press! A striking increase in gamma/delta T-cells in subcutaneous panniculitis-like T-cell lymphoma (SPTCL) can be a major diagnostic pitfall—one that might lead to confusion with primary cutaneous gamma delta T-cell lymphoma. Recognizing this pitfall is crucial to avoid misclassification. A must-read for fellow and enthusiasts! 😆🔬

🔥 Hot off the press! TROP2 is one of the hottest biomarkers in oncology, especially with TROP2-directed antibody drug conjugates now available. Did you know that cutaneous squamous cell carcinomas and certain adnexal carcinomas—such as porocarcinoma, hidradenocarcinoma, pilomatrical carcinoma, and squamoid eccrine ductal carcinoma—show high TROP2 expression? Check it out if you love and . The article is free for 50 days. 😆🔬
