Grade 3 Gynecomastia Surgery: What to Expect
Here's what severe gynecomastia actually requires, and why a multi-step surgical approach is the most reliable solution 👇
➡️ Grade 3 gynecomastia is the most severe form of male breast enlargement, marked breast tissue growth combined with significant skin excess and visible droop (ptosis). It almost always requires a combined surgical approach: gland excision + liposuction + skin removal with ni**le-ar**la repositioning to restore a flat, masculine chest contour.
What grade 3 surgery typically includes:
✔️ Liposuction: removes excess fatty tissue across the chest and lateral areas
✔️ Gland excision: removes firm, dense glandular tissue that won't shrink with diet or exercise
✔️ Skin removal: tightens the skin envelope when skin can't snap back on its own
✔️ Nipple-ar**la repositioning: moves the ni**le to a natural, masculine height on the chest wall
✔️ Compression garment: controls swelling and helps skin adhere to the chest wall post-op
⚠️ Grade 3 scars are a planned tradeoff, periar**lar, inframammary, or horizontal chest scars are expected and mature significantly over 6–12 months. Skin cannot be removed without leaving a scar; the goal is placing them where they fade best.
