2. Please include your contact information and a contact for the family.

Winners Announced: Monday, November 24, 2025, by 4:00 PM

Guidelines/Fine Print (for transparency):

• Gift cards will be distributed by Evans Funeral Home on the Monday of Thanksgiving week.
• The gift card will be valued at $100.00 and can be used at Walmart.
• Open to local families (must be able to meet in person to receive the gift card. Gift card will not be mailed or electronically transferred).
• Nominations can be submitted by friends, family, or community members.
• All nominations will be reviewed and kept confidential, however, winners will be announced on our page, after they have been contacted.
