Vascular surgery is a game of chess, not checkers.

The video shows what I did (a 2-stage basilic transposition), but the real story is why. Every decision we make—starting at the wrist, preserving the upper arm, choosing a 2-stage approach—is about protecting the patient's future.

We aren't just creating access for tomorrow. We are trying to ensure they still have options 5, 10, or 15 years down the line. We accept the extra work of a second stage now to buy them longevity later.

Before and After Revascularization for Peripheral Arterial Disease. I love giving each one of my patients minimally invasive options and reserving open revascularization when it’s really needed.

Having a clear plan is key in Vascular Surgery. I often use ultrasound as my initial diagnostic imaging tool while I am working up my patients.

Fistula vs. Graft. Why is one the "Gold Standard"?

It comes down to longevity and safety.

An AV Fistula (using your own vein) is almost always our first choice. Why? Because grafts carry a significantly higher risk of infection and thrombosis (clotting). A fistula is durable, natural, and requires far fewer maintenance procedures to keep it running.

But not everyone has the anatomy for a fistula.

In my latest YouTube video, I walk you through the specific anatomical requirements and the decision-making process when the "ideal" option isn't available.

The two biggest threats to your kidneys?

They aren't rare diseases. They are High Blood Pressure and Diabetes.

Uncontrolled, these two conditions aggressively accelerate Chronic Kidney Disease (CKD). But knowing the risk is only half the battle.

In my latest YouTube video, I dive deeper into the specific management strategies that can help halt this progression before it's too late. This is the deep dive your health deserves.

Everyone talks about what to pack for your Thanksgiving travel. As a vascular surgeon in Houston, let's talk about what happens to your body in that pressurized tube.

It's a 3-hit combo that creates a perfect storm for a dangerous blood clot (DVT):

1️⃣ THE SEAT (Immobility): Prolonged sitting completely shuts down your "Second Heart" (your calf muscles). Blood flow in your legs becomes stagnant.

2️⃣ THE AIR (Dehydration): The cabin air is drier than a desert. This dehydrates you, making your blood physically thicker and sludgier.

3️⃣ THE PRESSURE (Dilation): Lower cabin pressure can cause your veins to widen (dilate), making their tiny valves less efficient.

This 3-hit combo is a high-risk event for everyone, but it becomes non-negotiable to manage if you are over 40, pregnant, or have varicose veins.

Your flight plan is simple. SAVE THIS.

✅ WEAR compression socks (15-20 mmHg).
✅ DRINK 8oz of water per hour (the Aerospace Medical Association rule). ✅ MOVE your feet (30 ankle pumps every 30 minutes).

The #1 complaint I hear about compression socks? "They dig into my skin."

That's why I personally recommend .

They nailed the design. The top band adjusts to your calf without creating that painful tourniquet effect behind the knee. I wear the 20-30 mmHg knee-highs in the OR because they offer true medical-grade support without sacrificing all-day comfort.

Plus, the styles are actually fun.

Let's settle the debate: Are you Team Knee-High or Team Thigh-High? Drop your vote below! 👇

Everyone fears an aneurysm rupturing. But there's a second, hidden danger.

The turbulent blood flow inside that "bulge" can form clots. Those clots can then travel to your legs, cutting off circulation and causing severe ischemia. This silent killer has two ways to attack.

A simple ultrasound screening is life-saving.

Screening is recommended for men 65-75 with a smoking history. Tag a loved one. This simple scan could save their life. 👇
