About Focused Care at Humble

Focused Care at Humble is the local leader in post-acute services recognized by Medicare as a 5 star community, the only one in the area. The facility is furnished with a designated Transitional Care Unit, long term care suites and a specialized wound care program. The Transitional Care Unit encompasses a hall of fully furnished private suites equipped with flat screen televisions, half bathrooms and wireless internet connection. The updated Wandergaurd system supports our memory care capabilities. We provide 24 hour medical care by licensed nurses and physicians. At Focused Care at Humble our Medical Director, secondary physician and their appropriate staff practice in-house providing comprehensive and accommodating care. We are proud to offer a Wound Care Management program supported by a full time specialized nurse and physician. It is an honor to be comprised of a staff that has longevity of twenty-five plus years reflecting experience and confidence. Accompanied by our rehab team we offer physical, speech and occupational therapy. If long term care or therapy services are not appropriate for a patient’s needs, we also provide respite stays with private and semi-private suites available. Here at Focused Care at Humble we celebrate therapy graduations, birthdays, personal milestones and present daily activities for all to enjoy. Events we host include holiday parties, networking events, a health fair and prom in the spring. Excited to announce the expansion and renovation of our facility, we invite everyone in for a tour and an opportunity to introduce our community.
