Lindsey Vonn almost did the unthinkable… but unfortunately things didn’t go her way.

Here is what she had working against her:

1. #1 risk factor for a new injury is a previous injury to the same body part

2. The body automatically favors the non injured side through compensation

3. She didn’t have enough time to rehab/train her L knee to operate without an ACL

- heat or ice
- PLENTY OF REST

Then once you get back out there it tightens back up or you tweak it as soon as you run full speed….🤦🏽‍♂️

It’s time to shift your focus during rehab on building up your hamstring with a heavy emphasis on eccentric strengthening. Most athletes can accelerate at a high level, but struggle with the deceleration component required when moving at high velocities.

How’d we get here?
1. Minimal pain and swelling early on improving his ability to regain ROM and quad activation early
2. He has been compliant, diligent, and consistent since day 1
3. He has shown the prerequisite strength and stability to begin jogging regardless of the timeline

If you just had achilles surgery and you can’t do this, your ankle rehab failed.

But here’s the good news: it’s fixable. Film yourself doing the wall test on both sides. If the surgical side is more than an inch behind your non-surgical side, show your PT, and ask what they’re doing to close that gap.

Progress starts the moment you stop guessing and start measuring.

Here is a typical session for an ACL patient in the deceleration phase…

First we make sure that we’re not working out on a stiff knee by starting with mobility drills.

Second we start activating muscle groups that will be worked and are important for the main exercises in the session.

Next we add in our deceleration drills to ensure she is not fatigued while learning these new movement patterns.

Following we did some running intervals, because although she started running in the last phase we need to make sure she is getting her cardio up and improving her form.

Lastly we worked on strength.. picked 3 major movements to continue working on her weaker areas (quad, hamstring, and calves).

If your PT doesn’t have the answer … they don’t have a system that they’re confident in.

They are winging it or they waiting on the surgeon to tell them (but many time they are just looking at time anyway).

The person who clears you for sport should be looking at:
1. Your strength (measured with at least a dynamometer)
2. Your movement quality with running, jumping, cutting, landing
3. Your response and recovery to these movements
4. Your psychological readiness through testing
5. Your ability to decelerate on force plates

Following an Achilles repair there are certain measures I look at during each phase of rehab.

Before returning to sport my patients have to pass the triple hop test and land with a comparable deceleration rate of force development on the force plates.

In addition to this quantitative data they have to perform these movements with good quality before I will return them to sport.

Once you have achieved about 90% of your ROM following ACL surgery you should add these into your routine. They should be performed daily to prevent stiffness and pain while you’re progressing through the later stages of your rehab👌🏽
