We are a new home-based healthcare program focused on enhancing and extending the lives of those dealing with chronic, serious illnesses who are not yet eligible for Hospice care.
The Tri-State LifeCare (TLC) program provides home-based palliative medical care to patients of all ages within our service area while working in collaboration with each patient’s primary care physician. Our team is comprised of board-certified Palliative Care physicians with extensive experience in geriatrics, along with nurse practitioners with Palliative Care certifications, assisted by nurses and social workers.
Our caregivers see patients wherever their homes may be, whether a private residence, an assisted living facility, a senior group home, skilled nursing facility or other location. Having a home-based team offers a greater understanding of each patient’s situation and results in a more personalized plan of care.
The goal of palliative care is to help people with serious illnesses feel better, maintain their dignity and experience an improved quality of life. TLC services are provided in addition to disease treatments the patient may be receiving. We want to bring peace of mind and confidence to those grappling with difficult health issues, enabling them to spend more quality time at home with loved ones.

Tri-State LifeCare is a non-hospice program affiliated with Hospice of Huntington.
Our service area includes Cabell, Wayne, Putnam, Lincoln and Mason counties in West Virginia, and Lawrence County, Ohio. Tri-State LifeCare accepts most insurance.
