Synergy Support Services is dedicated to providing person-centered support to adults and children with developmental and intellectual disabilities. We are currently seeking candidates to assist with providing supports to individuals where they live, work and enjoy life!
JOB DESCRIPTION:
The Direct Support Professional is responsible for a variety of duties that focuses on supporting individuals with intellectual and developmental disabilities who receive services from the Bureau of Developmental Disability Services (BDDS). This includes the direct support of individuals and implementation of the person-centered individual support plans (PCISP). Direct Support Professionals are required to perform all duties in accordance with the State of Indiana regulations and Synergy Support Services policies and procedures.
DUTIES AND RESPONSIBILITIES:
1. Daily Programming
Assist individuals in the pursuit of recreational, social, and community activities based on their preferences and individual support plans;
Promotes skills and supportive behavior that guides individuals to accomplish their self-identified goals for living a good life.
2. Activities of Daily Living
Assist and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating;
Teaches skill development and supports individuals in the planning and preparing of nutritional meals based on individual needs and preferences;
Supports individuals with daily skills to care for their living environment including household chores, laundry, cleaning, and washing dishes.
3. Communication
Observes individuals for evidence of injury or changes in emotional and physical status;
Reports all medical-related incidents to the Client Services Coordinator or the delegating nurse;
4. Safety
Promotes awareness of the environment and self while encouraging other safety practices while on community outings.
Follows safety protocols to foster a safe working environment; All other duties as assigned;
JOB REQUIREMENTS:
Reliable transportation,
Active & Verifiable Automobile Insurance;
Active & Verifiable Vehicle Registration;
Active & Verifiable Driver’s License
Ability to lift/push/pull 50 lbs.
Ability to work on holidays and during inclement weather.
CPR Certification
