POSITION OVERVIEW
LVN to assist individuals with developmental disabilities by administering/dispensing medications and providing nursing care as established in their Individual Service Plan. A LVN may provide medical support and services to clients in their own homes or our residential facilities. An LVN, also acts as an advocate for the disabled individual, in communicating their needs, self-expression and goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Assist individuals with developmental disabilities in the Supported Living Services and Residential department.
Implementation and continuing evaluation of the Clientâs progress in accordance with the patient care plan while communicating with LVN team, Clinical Director, and Direct Care Staff of Client.
Collecting, recording, and monitoring patient data, routine measurements, and vital signs.
Administering insulin injections as ordered by the Clientâs physician in accordance with the patient care plan provided.
Monitoring blood glucose levels as needed to maintain healthy reads and communicating necessary information to the Client, LVN team, Clinical Director, and Direct Care Staff.
Monitoring the Clientâs diet to maintain healthy blood glucose levels and communicating this information to the Client, LVN team, Clinical Director, and Direct Care Staff of client.
Administer all ordered medication as needed including injections and treatments.
Inform Clients regarding any vital health information that may arise.
Charting in the Clientâs specific binder.
Assist in monitoring/educating Management and Direct Care Staff to ensure that all Clientâs medical needs are met.
Ensures that medication supply is adequate including prompting staff if medication refills are needed.
Commute to each Client location as needed to provide the required service.
Projects a positive and professional image.
Leadership - Maintains a positive, upbeat role in the Company. Promotes and exemplifies Company values and represents departmental objectives and interests to Management, Clients and Direct Care Staff.
Follows up on complaints, questions, and concerns that are brought to the attention of the LVN. Responds to Client needs in a timely and efficient manner. Develops and promotes teamwork and cooperation among fellow Employees, Managers, and Clients.
Shows an appropriate sense of urgency in assisting with Client needs that may arise.
Other duties as assigned by the Clinical Director.
Requirements

Education and/or Experience â
CPR Certified
Current California LVN licensure
Previous experience in a Behavioral Health industry a plus.
Required Technical / Other Skills/ Abilities-
Assist Clients and meet immediate medical needs.
Provide health information to Client, Direct Care Staff, Management, and Clinical Director.
Ability to determine health of Client upon assessment of visit.
