First, a home care assessment is conducted with a visit to your loved one’s home and a recommendation of services based on your needs and personal factors. Each Salus client is assigned a case manager, who acts as the home care team leader. The case manager is in direct contact with your loved one, supervises the caregivers, and serves as the focal point of communications between Salus staff, the patient and the family. Our approach as a home care agency is to offer a blended, integrated approach to providing for the medical and non-medical needs of our clients and patients.
