At the Jackson Care Hub, we work hand-in-hand with Michigan 211 to connect community members to vital resources like housing support, food assistance, transportation, and more. We invite all our partners to make 211 part of their everyday work by sharing this resource with clients, patients, and neighbors who need help.

A referral is made through the Hub to a local partner who can help. That partner accepts the referral, reaches out, and closes the loop so no one is left waiting and wondering.
