Geriatric care management consists of the process of planning and coordinating care of the elderly to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible. It entails working with seniors and their families by combining the knowledge of physical health, the aging process, family dynamics, and public and private resources. Geriatric

care management integrates health care and psychological care with other needed services such as: housing, home care services, nutritional services, activities of daily living/socialization as well as addressing financial and legal needs. Our Services Include:
