The Arc of Clarke County, Inc (TACC) is accepting applications for our full-time Health Care Coordinator position in Jackson, Alabama.
QUALIFICATIONS:
Required: RN or LPN (current Alabama licensure required)
*Valid Alabama driver’s license; insurable under employer’s insurance plan
*Must be able acquire CPR and First Aid Certification
*Excellent writing skills and the ability to work collaboratively with others
*No convictions of abuse, neglect, mistreatment, or exploitation involving care of others
*At least 21 years of age
Preferred:*Knowledge of social services and Medicaid Waiver programs
*Experience in supports for people with intellectual and developmental disabilities
*Previous supervisory experience preferred
*Ability to work with minimal supervision
RESPONSIBILITIES:
*Oversees the daily health care needs for people receiving nursing services
*Coordinates the development and implementation of medical plans of care
*Must be able to acquire Medication Assistance Supervising (MAS) certification
*Supervises all MAC workers
*Provides training to MAC staff to ensure implementation of medical plans
*Provides monthly progress notes and annual summary reports for people supported
*Complete annual nursing assessments for people supported
*Must be able to take call as required
This position is governed by the Nurse Delegation Program under the Alabama Board of Nursing.
This position is a full-time 40-hour flexible schedule with excellent benefits such as Blue Cross Blue Shield health insurance (95% is covered by the agency), paid holidays, paid annual leave, paid sick leave, retirement, on-call pay, and other incentives.
