1. Recording pertinent information pertaining to the client’s care.

2. Reminding clients to take medication at the appropriate time.

3. Planning and preparing meals according to specific dietary requirements of the client, and if necessary, feeding the client.

4. Assisting the client with therapy exercises under the direction of a Registered Therapist.

5. Assisting or performing personal care including hair care, oral hygiene, bathing, grooming, and dressing.

6. Assisting with ambulation with or without mechanical aids.

7. Assisting with routine bodily functions: Example, toileting.

8. Maintaining a safe, clean and healthy environment through light housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry.

9. Providing companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments.

10. Grocery shopping or other errands when needed.

11. Performing other housekeeping tasks as indicated in the care plan.

12. Taking client’s vital signs and recording input and output.

13. Observing and reporting changes in client's condition to this agency's Administrator.

14. Accurately preparing daily records and submitting them to the office by date due.

15. Participating in in-service education programs.

Providing great patient care.
Answering patient calls and determining how best to help them.
Providing physical support for patients or residents with daily activities and personal hygiene, including bathing, dressing, getting out of bed, - using the toilet, walking, standing, or exercising.
Turning and repositioning bedridden patients.
Ensuring patients or residents receive appropriate diet by reviewing their dietary restrictions, food allergies, and preferences.
Obtaining a wide range of information from physicians, caregivers, and nurses about patient conditions, treatment plans, and suggested activities.
Measuring and recording food and liquid intake and urinary and f***l output and reporting changes to medical or nursing staff.
Recording vital signs, including blood pressure, pulse, temperature, and respiration rate as requested by staff.
Examining patients to detect issues requiring medical care, including open wounds, bruises, or blood in the urine.
Reminding patients to take medications and nutritional supplements.
Noting observations of patient behavior, including complaints, or physical symptoms to nurses.
Stay up to date on CNA training and facility policy and procedure.

Performs homemaking activities which include (but are not limited to):
vacuuming, dusting, sweeping or mopping floors, doing dishes, changing bed linens, doing laundry and cleaning bathrooms.

Prepares meals and snacks according to instructions. May shop for groceries.
Provides companionship for the client. This includes accompanying client on walks, trips to social/recreational activities, assistance with hobbies, etc.
