🩺 Connecting uninsured residents of Duval County to no-cost and low-cost Primary Care.

JaxCareConnect is a joint collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents in finding a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to health care in order to support best possible health outcomes. Navigating health care can be an overwhelming process. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a health care provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their health care journey. JaxCareConnect aims to better inform uninsured neighbors about their health care options so that all Duval County residents have access to comprehensive primary care—regardless of insurance status or ability to pay. JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.

Every day, uninsured residents in Duval County face barriers that go far beyond medical bills — they impact dignity, independence, and hope.

Through JaxCareConnect, we’re helping two women overcome those barriers:
💜 A 21-year-old woman with cerebral palsy who needs a $2,000 dental procedure under anesthesia.
💚 A 46-year-old single mother whose broken scooter seat has left her unable to move freely or care for her disabled son.

Together, we can raise $2,500 to restore their health and independence, and every dollar is matched with an extra $0.25.

At JaxCareConnect, we know that access to nutritious food is essential to good health. The recent changes to SNAP benefits have created new challenges for many families in our community, but there is help available.

We’re grateful to and the many partners who came together to launch the Duval Care Coalition, a new community resource connecting residents to food support and other essential services.

If you or someone you know has been affected, visit jacksonville.gov/duvalcarecoalition to find local food distribution sites, learn about available resources, or discover ways to help others in need.
