A long-term minimal support program to assist an individual in maintaining employment. An individual is generally placed in this program through a Program Manager through the Human Service Center. How to access services
Individuals placed in Triumph’s Career Development Program would be referred through their Vocational Rehabilitation Counselor or their Program Manager at the Human Service Center. Day Habilitation
Our goal for this program is to encourage individuals to learn home living skills, work skills and social skills. Learning these skills promotes independence in their home, social network, and community. Residential / In-Home Support
Our goal for this program is to provide flexible, individually-tailored support service to individuals and families. Residential Habilitation
A residential service offered to assist individuals to maintain independence in their home. Services could include but not limited to: personal cares, meal planning, laundry, etc. Independent Habilitation
A residential service offered to assist individuals to maintain independence in their home. Hours are generally less than those recieved in residential habilitation services as less assistance is needed.

Individuals placed in the Residential/In-Home Program would be referred through their Program Manager at the Human Service Center.
