It brings us great sadness to announce that our Jefferson location will be permanently closed as of April 28th, 2022. Our Jefferson staff will be transferred to our Stayton location, and will continue to provide you with the top quality care you have come to know in Jefferson. On behalf of all of us here at Stayton Family Practice, we thank you for 13 wonderful years of operation and look forward to continuing you and your family's care at our Stayton and Lebanon branches!

It brings us great sadness to announce that as of April 28th our provider Troy Nies, PA will no longer be with our practice. If Troy is your current provider you may establish care with any of our other providers. We wish Troy the best of luck on his next endeavor! Our Stayton office has recently changed locations; their new address is 1881 Washington St. Stayton, OR 97383. If you are in need of immediate assistance feel free to call 503-400-6140 to schedule an appointment at either our Lebanon or Jefferson locations.
