We hear this all the time as pelvic PTs :

“I have tight hip flexors!” Or “I always have to stretch my psoas!”

When we test the length of the hip flexor (the pesky psoas), it’s not actually tight. It FEELS tight because it needs strength!!

When hip flexors are weak, it puts extra work on the muscles around it - like the diaphragm, lower back muscles to support the spine and the pelvic floor!

Sometimes the secret to tight hip flexors is strengthening it!

And postpartum? None of that automatically snaps back into place.

They’re meant to work as a team to manage pressure, support your spine, and keep you continent when you breathe, lift, laugh, sneeze, or move.

✨ Abs that only brace but don’t breathe?

Most people are told these are separate problems, so they chase separate fixes.

But when we zoom out, there’s often one common thread: the pelvic floor.

Because the same muscles help control p*e, p**p, and support your spine, dysfunction can show up in ways that feel totally unrelated. And research backs this up, pelvic floor rehab is recommended as first-line treatment for leaking, constipation, and pelvic pain… not a last resort.

Prolapse doesn’t happen because your body failed, and it’s not caused by childbirth alone. Vaginal birth is a risk factor, but also chronic coughing and chronic constipation causing strain to important fascial and ligament structures is what ultimately causes prolapse.

Pelvic floor weakness is not a causative factor, either! However, a weak pelvic floor CAN cause more stress to already stressed out fascial and ligament structures. The pelvic floor is designed to take the pressure off our ligaments - just like all muscles in our body are designed to reduce strain to our other tissues!

A diagnosis of prolapse is nothing to fear and surgery is not always needed.
At Break Free Pelvic Health, we can assess your tissues to determine if you will be successful with pelvic floor physical therapy alone. Sometimes surgery is needed. Sometimes vaginal estrogen is needed. And sometimes a pessary would be life changing! We help to strengthen the pelvic floor to take stress off your tissues, manage and treat constipation, teach you how to lift and cough with pelvic floor support, and more. If you need a pessary, we fit those too!
