📢 Important Update for Our Patients
We’re transitioning our patient portal to a new system to better serve you. Soon you’ll be using FollowMyHealth to access your health information with us at Elemental Care – Health & Wellness Center, LLC.
This change was mandatory and required by Practice Fusion, our electronic health record provider. It is not something we can opt out of, but we’re here to make the transition as smooth as possible for you.
Here’s what you need to know:
• You will receive an email invitation to create or link your FollowMyHealth account.
• Once our switch is complete, your old Patient Fusion login will no longer work for accessing records, messaging, or appointments.
• With FollowMyHealth you’ll be able to view lab results, message your provider securely, see upcoming appointments, and more—all in one platform.
• Please ensure your email address listed with us is up to date so the invitation reaches you.
📆 What this means for you now:
• If you have a current account with Patient Fusion, check your email for the invite from FollowMyHealth.
• If you don’t receive the invite within a few days, contact our office and we can resend or troubleshoot.
• After the switch, all portal access will be via FollowMyHealth—please bookmark it and update any saved links.
