This patient presented with a scalp scar following excision of a prior tumor.

Given the characteristics of scar tissue, we opted for hair transplantation using follicular units harvested from the posterior scalp.

The recipient site was prepared with autologous fat grafting a few weeks prior to transplantation to support vascularization and improve tissue quality.

Each graft was then individually selected and placed, respecting direction, angle, and density to optimize integration and achieve a natural result.

In scar tissue, careful planning and attention to detail are key determinants of successful outcomes.

Have you ever wondered about what happens right after an FUE procedure? Dr. Takata had a follow up appointment from a patient who had hair transplant surgery the day before. He returned for the standard next-day wash.
The donor area already looked clean; those tiny extraction spots usually heal in 2–3 days and soon fade. The recipient area will have crusts for about 10–15 days. After that, some shedding can occur, but the graft bulbs are still there. Expect the first hairs at around three months, a clearer result at six months, and the final outcome at one year.
Sharing these steps and timelines with patients makes recovery less stressful. What part of the post-op timeline do you find most surprising?

Our patient came to the office for a 6-month follow-up after a hair transplantation.
In this case, he received a total of 4,500 grafts to achieve this level of density and coverage. Dr. Mejia performed a Follicular Unit Transplant (FUT) procedure to rebuild his hairline and crown.

Now that the hair has grown in, you can see how much thicker and more natural his hair looks from every angle. Even with an FUT procedure, a specialized "trichophytic closure" technique ensures the donor scar is virtually undetectable, even when the hair is combed through.

Dr. Mejia recently attended the Live Surgery Workshop in Pakistan hosted by the Hair Restoration Society of Pakistan in Islamabad, Pakistan, which is a Global Council member of the ISHRS.  He was honored with a plaque as Past President of the ISHRS as well as cutting the ribbon in the opening ceremony for the event. But mostly, as always, it's the collegiality and continual learning and sharing ideas that drive him.

Dr. Luciana Takata performed an eyebrow hair transplant last week on a patient that had very thin brows since she was young and has been doing micropigmentation for many years. She was ready for something different. . The patient’s hair was extracted from the back of her head and the hair grafts directly implanted into the eyebrow. Here is a sneak peek during the surgery. Last week, I spoke at the American Academy of Cosmetic Surgery Annual Scientific Meeting in Tampa, Florida at the Hair Restoration session. While it was a while ago that I attended one of the AACS meetings, I really enjoyed learning new perspectives in the field of cosmetic surgery especially regenerative treatments and hey, I might even attend their facial cadaver course coming up to learn new techniques in the anatomy of the face.

A highlight of the event was meeting Dr. Underwood who is the incoming President of the American Medical Association. I loved his energy and ideas to work together to advance the field.

The Gala was held at the Tampa Aquarium, which was totally in my element since I have a descent sized aquarium at home, but obviously not with sharks and loggerhead turtles.

The comraderie was very welcoming and they openly shared innovative ideas. I met new friends and hope to attend again!

As the 2024–2025 President of the , a global non-profit Society, I had the privilege this past year of traveling around the globe, not just to speak and teach, but to spend time with friends and colleagues who share a deep passion for hair restoration and patient care. Together, at live surgery workshops across many countries, our members collectively moved and placed millions of hair follicles, advancing our field one patient at a time.

What made this year truly special was the collegiality, the conversations in operating rooms, lecture halls, and over shared meals; the generosity with which knowledge was exchanged; and the genuine friendships that continue to define the ISHRS. Across borders and cultures, the Society remains united by mutual respect, collaboration, and a shared commitment to excellence.

I am incredibly proud to have served as President with a guiding theme of Unity, and deeply grateful to call so many of you not just colleagues, but friends.

Dr. Takata demos the Hair Matrix Device live on a patient. This device is a detailed AI-assisted analysis that measures hair density, miniaturization, and scalp condition across multiple areas including the frontal region, temporal zones, and crown. This gives us an accurate baseline before starting any hair treatment plan.

In this patient’s case, the evaluation showed mostly terminal hairs and minimal miniaturization, meaning there is no evidence of androgenetic alopecia. So the suggested treatment is to start Minoxidil and possibly other non surgical treatments such as the Ted Ultrasound.

Lichen Planopilaris (LPP) is an autoimmune condition that causes inflammation and scarring of the scalp often beginning along the frontal hairline. Over time, this inflammation can destroy hair follicles, leading to permanent hair loss if not treated promptly.

At this stage, for this patient, Drs. Mejia and Takata do not recommend hair transplant surgery. Instead they will focus on calming the scalp.

Various treatment options for this type of condition may include topical or injected steroids, dutasteride, red light therapy, platelet rich plasma (PRP) or other medical agents.
