Actually… I hope you do take offense.

Because this isn’t about egos…
this is about people’s lives and function.

Some of you are either:
• too lazy
• too burnt out in a system you shouldn’t be in
• or simply not giving patients the care they deserve

And that matters.

Because when you miss the basics, you’re not just affecting one person,
you’re affecting their ability to walk, work, play with their grandkids, show up for their spouse and be present for their family.

And this rant isn’t random.

This comes from seeing patients at one of the most expensive communities in the world,
people who have access to the best care money can buy on the mainland.

They come here on vacation…
and I end up being the one telling them:

“You’re behind.”
“You don’t have knee extension.”
“Key pieces of your rehab were missed.”

That shouldn’t happen.

At that level? With those resources?

It makes me question our profession more than I’d like to admit sometimes.

No one is asking you to be elite.
No one is asking you to be a high-performance sports specialist.

But if you can’t handle a basic knee replacement…
if you’re not prioritizing knee extension…
if you’re missing the most fundamental piece of the puzzle…

Then yeah, take offense.

Because that’s unacceptable.

There’s no reason someone 6 weeks post-op should have never worked on knee extension.
That’s not complicated.
That’s not advanced.
That’s day one stuff.

And to the doctors pushing apps and handouts instead of real rehab
do better too.

Some patients can navigate that.
Most can’t.

They need guidance, intent, and understanding.

Because going through the motions isn’t rehab.
Ex*****on is everything.

Can we all just do a little better in healthcare

Blood Flow Restriction (BFR) training is a highly researched tool that if your PT clinic isn’t using 🚩🚩🚩

By partially restricting venous blood flow, we create a high metabolic demand with low loads.

Meaning:
→ Less joint stress
→ More muscle activation
→ Faster strength gains when heavy loading isn’t an option

This is how we bridge rehab → performance.

Train the system… not just the symptom.

Stability isn’t stillness. It’s control in chaos.

If your rehab is only slow and perfect, it’s incomplete.
Sport isn’t controlled it’s cuts, fatigue, contact, and unpredictability.

If you don’t train it, don’t expect it to magically show up.

I see it every day. The gap between rehab and sport. Sometimes it’s an insurance issue and others times it’s just a poor clinician.

But you have been educated so advocate, ask questions, and seek quality care.
