As a therapist for 25 years, I’ve had a full career listening to clients about their coping behaviors and providing non-judgmental support in order for them to understand the function of their strategies, including eating disorder behavior. 

As I listen to the debate back and forth between the supporters of weight loss drugs and the naysayers, I find myself thinking of a very common question I ask my clients: 

What is the function of this use? 

Obviously GLP-1 use is satisfying our society’s constant pursuit of thinness through a quick, medical solution, however,  I’m also hearing women say it helps stop the “food noise.”
“Food noise” is a colloquial term used to describe what therapists call ‘intrusive thoughts’: upsetting, repetitive thoughts that can feel hard to manage.
My questions are:

What happens to the intrusive thoughts after you discontinue the use of GLP-1s?

Do you understand why your intrusive thoughts occur in the first place?

Do you understand how to manage intrusive thoughts once they arise so you can sustain your behavioral change?

There are evidence-based strategies to address intrusive thinking, which will lead to more sustainable health and weight management. 

If “food noise” is NOT assessed and addressed properly, the challenge will return when GLP-1s are discontinued. Frankly, I find it insulting that providers aren’t offering more effective solutions for patients that have chosen to use these medications.

As you begin your holidays, remember that nothing is ever black and white. Radically accepting things as they are enables us to effectively plan for them. A few prompts for managing holiday stress at family gatherings: • What do you plan to focus on at the event? • How can you manage vulnerability factors beforehand (eg don’t show up “hangry,” be well rested, do something relaxing or energizing before event) •Create a coping ahead plan for anticipated tension vs hoping it doesn’t occur •Use grounding techniques to interrupt triggering thoughts (eg I feel the warmth of my cup, I like the color of the green blanket, notice the flickering of a candle) •Tell someone close how you might need support •Have a practiced “exit” phrase to reduce potential consequences
