We're halfway through Patient Safety Awareness Week, and one of the ways Legacy Home Health Care keeps patients safe is through our Falls Prevention Pathway to Independence—providing fall prevention education, home safety assessments, medication reviews, and personalized exercises to improve strength and balance.

This year’s theme is “Team Up for Patient Safety,” and we’re incredibly grateful for our dedicated clinicians and staff who partner with patients and families every day to keep safety at the center of care.
