but your HEAD is
the PROBLEM!!

FACT #1 Our HEAD weighs a lot.
FACT #2 Everything is connected.
FACT #3 Our brain will do whatever it needs to do to prevent us from falling forward.

TRUTH: If our head is too far forward and our bodyweight gets shifted from heel to toe, our brain will ask our DEEP GLUTE muscles (not the one's you can see!) to "turn-on" in order to prevent you from falling forward. That is a lot of stress for four little muscles that attach from your tailbone (sacrum) to your leg.

SAD TRUTH: This stress can ended creating low back, hip, and even knee pain.

EVEN SADDER TRUTH: Many people will never realise that the HEAD is the problem. As a result, their pain becomes chronic.

ADVICE: Always think, "My pain is NOT the problem." Learn to use your posture as guide to help you move and feel better.

Why is the psoas muscle so important in patients with chronic pelvic pain?

The psoas may be considered an accessory core muscle which supports the lower spine pelvis and hips. The psoas is made up of the psoas major and psoas minor and joins the iliacus muscle. We can then call it the iliopsoas muscle. The psoas attaches to the spine at T12 to L5. It inserts into the minor trochanter of the femur and runs over the p***c ramus. The iliopsoas functions to flex the hips and trunk. The deep hip rotators connect to the hip just above the psoas and therefore tightness in the deep rotators can effect the iliopsoas and pelvic floor muscles. With tight deep hip rotators the relationship between the pelvic floor muscles (or pelvic diaphragm) and respiratory diaphragm is interrupted. This disruption weakens the abdominal muscles due to its relationship with the respiratory diaphragm. This can result in pelvic instability, sacroiliac joint dysfunction and having less support of the lumbar spine. Thus, it is important to treat the psoas using soft tissue mobilization, lengthening techniques followed by strengthening exercises. The kidneys and colon might influence the psoas and vice versa. Balancing the iliopsoas and other pelvic and hip muscles is necessary for patients with chronic pelvic pain. ,

Here are many known, treatable causes. Let's talk about a few: โ €

๐Ÿ’› The vestibule relies on estrogen and testosterone for normal functioning. Sustained hormonal deficiencies result in compromised integrity of the vestibule. Breast feeding occurs when estrogen is low enough to allow lactation, which is great for breast milk but not ideal for the v***ar health ๐Ÿ’Š Many birth control pills prevent pregnancy by suppressing ovulation with synthetic estrogen. Birth control pills also elevate our SHBG which reduces free testosterone; therefore, many types of birth control pills can be a 'double whammy' hit to the vestibule. In menopause our estrogen and testosterone decline, also leading to compromise of the vestibule.โ €

๐Ÿ’› The vestibule has a higher density of nerve fibers than other parts of the v***a. This helps enhance sexual pleasure but can also result in pain when sensitized. Repetitive infections can irritate these fibers over time and result in persistent pain in the vestibule after the infection is cleared. โ €

๐Ÿ’› Lichen sclerosus, planus, and other skin dermatoses can compromise v***ar and vaginal tissue and lead to pain at the vestibule.โ €

๐Ÿ’› Pelvic floor myalgia and tightness results in mechanical strain on the vestibule and commonly causes vestibulodynia in the posterior aspect of the vestibule. If a person has vestibulodynia from hormonal, infectious or disease they often also have pelvic floor dysfunction.

QUADRATUS LUMBORUM- Anterior view of the lumbar spine and lowest ribs reveals a complex system of soft tissues.
The Quadratus Lumborum is not a single plane muscle but rather a complex network of fibers and LINKS to other tissues. Specifically, the QL links the lowest ribs (costals), lumbar transverse processes and the ilium.
The QL has three layers:
Purple- Costo-Transverse (deepest)
Green- Costo-Iliac (middle)
Pink- Ilio-Transverse (superficial or most posterior)
The blue represents the Lateral Arcuate Ligament connecting the 12th rib to L1.
The lowest fibers of the QL actually become Ligament tissue in adults and become the Iliolumbar ligaments (orange and yellow). These ligaments connect L4 and L5 to the ilium and are often involved in low back pain.
All of these structures have a role in function and need to be considered in back pain cases.
