We’re proud to spotlight Melissa Zadorozny, one of our compassionate RN Case Managers in New Jersey. For the past 6 months, Melissa has been providing comprehensive, heartfelt care to our patients and their families during some of life’s most vulnerable moments.

Melissa’s role is rooted in dignity and connection. She coordinates care, manages symptoms, offers guidance, and ensures each patient’s plan truly reflects their goals, values, and comfort. What she loves most is the opportunity to bring peace, meaning, and comfort to families—helping them reminisce, celebrate small victories, and find beauty even in difficult times.

Our Swan Hospice team spent time with the Thomas family at CC Ocean Grove, and it was truly a beautiful visit. The holiday playlist filled the room with familiar songs, smiles, and shared memories — and everyone there enjoyed every moment.

🌺 Feature Friday: Meet Nicole Rafols 🌺

Nicole is one of our dedicated Medical Social Workers in New Jersey, and for the past 3.3 years she has been a steady source of emotional and practical support for our patients and their families. From counseling and end-of-life conversations to education and connecting families with resources, Nicole plays an essential role in guiding others through some of life’s most challenging moments.

When asked what she enjoys most about her work, she shared:

💜 Feature Friday: Meet Kathy Donadio 💜

Kathy is our incredible Volunteer Coordinator in New Jersey, and for the past 5 months she has been the heart behind recruiting, training, and supporting our dedicated team of volunteers. She ensures every volunteer is prepared, confident, and matched with meaningful roles that fit their strengths and passions. Kathy also works closely with our hospice staff to make sure volunteer support reaches the patients and families who need it most.

When asked what she enjoys most about her role, she shared:
