📢 On October 31, 2025, CMS released the 2026 Physician Fee Schedule Final Rule, outlining new Medicare Part B Therapy Service rates and policies effective January 1, 2026. Key updates include revised CPT rate calculation methods with an efficiency adjustment and a separate conversion factor for APM participants (excluding SNFs), updated KX Modifier thresholds of $2,480 for OT and PT/ST combined with continued Targeted Medical Review above $3,000, and changes to telehealth rules. PT, OT, and ST providers are no longer approved telehealth practitioners, and CMS advises issuing Advance Beneficiary Notices (ABNs) for telehealth services after October 1, 2025.
