📢 CMS has released significant updates impacting SNF Quality Measures. The new MDS 3.0 QM User’s Manual v18.0 (effective Jan 1, 2026) includes major changes to the Antipsychotic Medication Long-Stay Measure, now incorporating Medicare/Medicaid claims, encounter data, updated diagnosis code requirements, and a new Hospice exclusion. Updates to the Discharge Function Score reflect the removal of OT/PT minute items, shifting risk adjustment to the new O0425 items. CMS also released updated Falls with Major Injury Measure technical specs, expanding the measure to include claims data from hospitals, ED, and observation stays.
