Since 1976, we’ve walked alongside people with disabilities to open doors, build skills, and transform the way our community thinks about independence. That work is only possible because of friends, neighbors, and partners like you.

When you give, you’re not just donating. You’re helping ensure no one is left behind 💙
Be part of the story of independence and inclusivity. You can donate through this link: https://www.dncap.org/donate/

Every new chapter in life comes with change. And change, while powerful, can be tough to navigate.

You might be moving from high school into adulthood, leaving a nursing facility to return to community life, or adjusting to a new disability; whatever it is, we offer Transition Services that are designed to guide, support, and empower you to take the next step toward independence.

We believe transitions aren’t just about what you leave behind, but about what’s possible ahead 💙
