A HUGE thank you to Acacia Springs Assisted Living for allowing Covenant Hospice Care to honor the very deserving Veterans within your community. There were many laughs, some tears and most importantly, appreciation and gratitude for the brave men and women who served in our armed forces. Each one of the veterans received a personalized certificate of appreciation, a star from an American Flag that can no longer fly from the starsforourtroops.org and an American Flag lapel pin. What an honor it was to be apart if this moment.

Covenant Hospice Care, Courtney Kaplan and Danielle Davis, and The Wentworth Assisted Living Community shared in honoring Veterans who gave their bravery and sacrifice to serve our country. Each of these Veterans received a certificate of thanks from the WE HONOR VETERANS program, Covenant Hospice and The Wentworth along with a lapel pin. The memories shared amongst the comrades was emotional at times. Many have not spoken about their service due to PTSD, moral injury and fear. You can't help but feel grateful and humbled when you are in a room full of these amazing heroes. Thank you!

Covenant Hospice Care provides an individualized program of physical, emotional, and spiritual care for people in the last phases of a life-limiting illness. Our program is reflective of a spirit and idea of caring that emphasizes comfort and dignity for the patient making it possible for them to remain independent for as long as possible, in familiar surroundings. Our company centers around the idea of a family lending support to each other where ever possible. Our mission is to enrich the life of our patients and their loved ones by providing meaningful end of life experience, compassionate care, comfort and peace.

The registered nurse plans, organizes and directs hospice care and is experienced in nursing with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.

Patient Care

*Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
Provides professional nursing care by utilizing all elements of the nursing process.
*Assesses and evaluates patient's status by writing and initiating plan of care, regularly reevaluating patient and family./caregiver needs, and participating in revising the plan of care as necessary.
Uses health assessment data to determine nursing diagnosis.
*Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
Initiates appropriate preventative and rehabilitative nursing procedures. *Administers medications and treatments as prescribed by the physician in the physician's plan of care.
Counsels the patient and family in meeting nursing and related needs.
*Provides health care instructions to the patient as appropriate per assessment and plan.
*Assists the patient with the activities of daily living and facilitates the patient's efforts towards self-sufficiency and optional comfort and care.
*Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Communication

*Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
*Communicates with the physician regarding the patient's need and reports changes in the patient's condition; obtains/receives physician' orders as required.
*Communicates with community related persons to coordinate the care plan.
Teaches the patient and family/caregiver self-care techniques as appropriate.
*Provides and maintains a safe environment for the patient.
*Assists the patient and family/caregiver and other team members in providing continuity of care.
*Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
*Attends interdisciplinary group meetings.

Additional Duties

*Participates in on-call duties as defined by the on-call policy.
*Ensures that arrangements for equipment and other necessary items and services are available.
*Supervises ancillary personnel and delegates responsibilities when required.
*Assumes responsibility for personal growth and development while maintaining professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
*Fulfills the obligation of requested and/or accepted case assignments.
*Actively participates in quality assessment performance improvement teams and activities.

Position Qualifications

*Graduate of an accredited school of nursing. One to two years of acute care experience in an institutional setting, and one to two years experience in home care.
*Current licensure in Nevada and CPR certification.
*Bachelor's degree, with one year of hospice care experience preferred.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients.
*Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
**Must have at least one year of home hospice experience.**

It was an absolute honor serving our veterans at US Vets today. Covenant Hospice is a participating member of the Hospice Veteran Partnership through We Honor Veterans program. Several local hospice's gathered much needed donations and helped serve a wholesome meal to outer local veterans receiving services at US Vets. A great big thank you to Krispy Kreme for the donuts, Platinum Catering for the delicious meal and Signal Healthcare for your generous donation. .
