Our Story
We provide a variety of services to all of our patients. This includes: ASSISTED LIVING FACILITIES AND GROUP HOMES We provide pharmacy services to many assisted living facilities and group homes throughout Las Vegas and Henderson. We work closely with the administrators and patients by sending the medications directly to the facilities. In addition, we provide special packaging (ex: bubble packs or bingo cards) for these patients to remain compliant with state regulations and improve patient medication adherence. We improve patient compliance with monthly refill reminder programs, as well as providing the facilities with MARs (medication administration record). We work closely with family members and provide billing statements monthly to their loved ones. HOSPITAL SERVICES - MEDICATION THERAPY MANAGEMENT (MTM) What are medication therapy management services? Medication therapy management (MTM) is a distinct service or group of services that optimizes drug therapy with the intent of improved therapeutic outcomes for individual patients. Services includes a broad range of professional activities, including but not limited to performing patient assessment and/or a comprehensive medication review, formulating a medication treatment plan, monitoring efficacy and safety of medication therapy, enhancing medication adherence through patient empowerment and education, and documenting and communicating MTM services to prescribers in order to maintain comprehensive patient care. Medication therapy management includes five core components: a medication therapy review (MTR), personal medication record (PMR), medication-related action plan (MAP), intervention and/or referral, and documentation and follow-up. What is a medication therapy review (MTR)? A systematic process of collecting patient and medication-related information which occurs during the pharmacist-patient encounter. In addition, the review assists in the identification and prioritization of medication-related problems. During the MTM encounter, the pharmacist develops a PMR for use by the patient. What is the benefit of receiving a comprehensive medication review? Medication-related problems and medication mismanagement are a massive public health problem in the United States. Expenses associated in treating medication related adverse events resulting in hospitalizations doctor visits, lab exams, etc., cost the economy over $200 billion every year. Patients are not immune and responsible for their copays, along with their general well being. Patients on average take 11.4 meds/day. It is a daunting task to manage, correctly administer, understand the role of medications, and understand the disease state. An estimated 12% of the U.S. population is considered to be health literate. What can I expect during an MTM session? A typical comprehensive review lasts about 30-45 minutes. A pharmacist will sit down with you and review every prescription and over the counter medication, including supplements and alternative therapies. Actively discussing and asking questions will enhance and strengthen your understanding, thus optimizing your medication regimen. The pharmacist will schedule a one week follow up by staff and provide a quarterly review to update medication list and follow up with patient. How do I sign up to receive services? The FDA approval process is intended for mass-produced drugs made by large manufacturers.
