LOREN PERRICONE-ISRAEL, LMSW JOINS EVERHOME CARE ADVISORS & VIVALYNX VIRTUAL HEALTHCARE

January 11, 2026 — EverHome Care Advisors & Healthcare Advocates is proud to announce the appointment of Loren Perricone-Israel, LMSW to the role of Professional Healthcare Advocate and Care Coordinator. In this position, Loren will play a pivotal role in advancing EverHome’s mission of guiding individuals and families through complex healthcare and home care decisions with compassion, expertise, and unwavering support.

Loren brings more than 14 years of experience in the human services and healthcare fields to EverHome Care Advisors. A Licensed Master Social Worker, she has developed deep expertise working with diverse populations across the lifespan. Her career includes serving as a medical social work care manager at Trinity Health St. Peter’s Medical System, where she focused on supporting patients 55 and older and advocating for the needs of older adults. Her work spans physical disabilities, mental health challenges, substance use disorders, and medically complex individuals, with a strong emphasis on enhancing quality of life and independence.

In her new role, Loren will be instrumental in providing personalized care guidance and advocacy services to EverHome’s clients. Individuals living with chronic illness, long-term disabilities, and aging populations who wish to remain safely and independently at home will benefit from Loren’s expertise. She will support families and care partners by conducting comprehensive assessments, helping develop individualized care plans, navigating healthcare systems, and connecting clients with crucial resources and services. Through collaborative care planning and dedicated advocacy, Loren will help ensure that every client receives the right care at the right time.
