Most women are told about hot flashes.

Almost no one talks about the other perimenopause symptoms.

Perimenopause can start in your late 30s or 40s, and the symptoms aren’t always what you expect.

Many women experience things like:
• Suddenly lighter or heavier periods
• Anxiety that feels new or unfamiliar
• Brain fog or trouble concentrating
• Random heart palpitations
• Joint pain or body aches
• Sleep disruption
• Mood swings or irritability
• Changes in how alcohol affects you
• Dry eyes, dry skin, or vaginal dryness

And because these symptoms seem unrelated, many women never realize hormones could be the reason.

Instead, they’re told it’s stress, aging, or “just part of life.”

It’s not in your head. And you deserve to understand what your body is going through.

Save this so you can recognize the signs  and share it with a friend who might need to see it.

If you have been told your labs are “normal” but you still feel bloated, exhausted, anxious, inflamed, and stuck with stubborn weight gain, it may be time to look beyond standard testing.

Certain functional labs can uncover patterns that basic blood work often misses in perimenopause.

A comprehensive stool analysis like the Vibrant Labs Gut Zoomer can reveal bacterial imbalances, yeast overgrowth, parasites, inflammation markers, and signs of increased gut permeability. This can help explain why you feel reactive to foods you used to tolerate, bloated after healthy meals, or unable to lose weight despite doing everything right.

The Hormone Zoomer by Vibrant Labs test provides a full picture of how estrogen, progesterone, cortisol, and androgens are metabolized throughout the day. Instead of a single snapshot, you see patterns. This can clarify mood swings, poor sleep, anxiety, and weight gain around the hips and midsection that feels impossible to shift.

Mycotoxin testing looks for hidden mold exposure, which can quietly disrupt hormones and metabolism. Brain fog, crushing fatigue, persistent inflammation, and unexplained weight gain are often brushed off as stress or aging when mold may be part of the picture.

An Organic Acids Test offers insight into mitochondrial function, neurotransmitters, detox pathways, and nutrient deficiencies. If coffee barely helps, workouts leave you wiped out, and your motivation feels gone, this deeper look at cellular health can be a game changer.

You are not crazy. And you are not failing. Sometimes you just need better data to understand what your body has been trying to say all along.

If your energy crashes mid morning or mid afternoon, you feel shaky when you wait too long to eat, crave sugar after lunch, wake between 2 and 4 AM, or feel wired at night but exhausted in the morning, your blood sugar may be out of balance.

Mood swings, brain fog after meals, puffiness, stubborn belly weight, intense PMS, and night sweats can also be tied to glucose spikes and crashes. These symptoms are common in midlife, but they are not something you have to accept as normal.

When blood sugar constantly rises and falls, cortisol increases, insulin gets overworked, and inflammation climbs. Over time, that pattern pushes the body into fat storage and survival mode, even if your labs look “fine.”

Start simple. Eat within 60 to 90 minutes of waking. Build every meal with protein, healthy fats, and fiber. Do not eat carbs on their own. Aim to eat every three to four hours to avoid the rollercoaster.

Small, consistent shifts in blood sugar support can dramatically improve mood, sleep, cravings, and hormone balance.

When hormones begin to shift in perimenopause and menopause, your nutrient needs shift too. This is a season where targeted support can make a real difference in how you feel day to day.

Vitamin D becomes especially important for protecting bone density as estrogen declines. It also plays a role in immune health and can support mood stability, which many women notice fluctuates in midlife.

Magnesium is another key player. It supports better sleep, helps muscles relax, calms the nervous system, and contributes to heart health. If you are feeling wired but tired, this mineral is often worth a closer look.

Omega 3 fatty acids are powerful for brain function and joint comfort, and they help regulate inflammation, which tends to rise during this stage of life.

Vitamin B12 supports steady energy, healthy nerve function, and mental clarity. If brain fog or fatigue has crept in, optimizing B12 can be an important piece of the puzzle.

And Vitamin E has been studied for its potential to ease hot flashes while also supporting skin and cardiovascular health.

The goal is not to take everything at once, but to understand what your body may need more of in this season. Personalized support, quality nutrition, and the right supplementation can help you feel more steady, clear, and resilient as your body changes.

Inflammation is one of the biggest drivers behind so many of the symptoms women experience in perimenopause and menopause. The tricky part is that it does not always show up in obvious ways. Instead, it often hides behind symptoms we are told are just “normal aging.”

If hot flashes and night sweats have ramped up, inflammation may be playing a role. Certain foods, chronic stress, poor sleep, and blood sugar swings can all increase inflammatory responses in the body, which can make vasomotor symptoms more intense and more frequent.

Noticing more joint stiffness or achiness lately? As estrogen declines, the body loses some of its natural anti inflammatory protection. That shift can leave joints feeling sore, especially in the morning or after sitting for long periods.

If weight gain seems to be settling more around your midsection, that is also common in this stage of life. Changes in hormones can increase visceral fat storage, and that deeper abdominal fat is closely linked to inflammatory activity in the body.

And if you are experiencing more vaginal dryness, inflammation may again be part of the picture. Hormonal shifts can alter the vaginal microbiome, which can contribute to irritation and dryness over time.

None of this means your body is broken. It means it is changing. When we focus on calming inflammation through supportive nutrition, stress management, strength training, and sleep, we often see meaningful improvements in symptoms. The goal is not perfection. It is creating an environment where your body feels safer and more supported.

Breast cancer care needs more nuance, more conversation, and more truth.

I’m honored to be part of the “Everything Breast Cancer” online symposium, a 2.5-day event bringing together 15 leading physicians to talk about what patients and providers aren’t always hearing.

You will hear about:
✨ Hormones & breast cancer (the real science, not the fear)
✨ Functional + standard-of-care integration
✨ Immunotherapy
✨ Vaginal hormone safety during treatment
✨ Nutrition as medicine
✨ Case studies & clinical application

Breast cancer rates are rising especially in younger women. At the same time, many women are left confused about hormones, contraception risks, and post-treatment options.

Knowledge is power.
Patients deserve informed conversations.
Providers deserve better education.

This event is online and available for replay all year.

If you’re a provider or someone who wants deeper understanding this is an important conversation to be part of.

If you have ever tried to “eat better” in midlife and immediately felt overwhelmed, you are not alone. Somewhere along the way, we were made to believe that supporting our hormones means overhauling everything at once. More plants. More protein. More planning. It can start to feel like managing food is a full time job.

The truth is, it does not have to be all or nothing. Instead of aiming for perfect, think about becoming more plant forward in simple, realistic ways.

You can stir red lentils into soups or pasta sauces for an easy boost of protein, fiber, and iron. They cook down beautifully and blend right in, which makes them perfect if you are feeding a family, too.

Swap in organic soy milk for your latte or smoothie to add quality protein that is comparable to dairy, plus beneficial plant compounds that may help with hot flashes for some women.

And keep shelled organic edamame in your freezer to toss into salads or stir fries. It is one of the easiest ways to add a satisfying mix of protein and fiber without extra prep.

Small shifts add up. You do not need a complicated meal plan. You need sustainable habits that support your body in this season.

Hormones play a measurable role in cardiovascular health, particularly in women. Estradiol supports healthy blood vessel function, regulates inflammation, and helps maintain normal blood pressure through its effects on the endothelium.

These protective mechanisms help explain why cardiovascular risk increases as estrogen levels decline during perimenopause and menopause.

Testosterone and progesterone also contribute to heart health in distinct ways. Testosterone supports cardiac output and muscle strength, while progesterone promotes blood vessel relaxation and improved blood flow.

Together, balanced hormone levels support vascular function, blood pressure regulation, and overall cardiac efficiency. Understanding these roles is essential when evaluating cardiovascular risk across the menopausal transition.

Hormone replacement therapy is not one-size-fits-all. Different hormone types vary in structure, origin, and how the body recognizes and responds to them.

Understanding the differences between bioidentical hormones, synthetic progestins, and equine-derived estrogens is essential for informed, individualized care.

Choosing HRT should be based on physiology, symptoms, risk profile, and goals,  not marketing or blanket recommendations. When patients understand what they’re taking and why, treatment becomes more precise, safer, and more effective.

Hot flashes are often treated like a simple hormone problem. Estrogen drops, symptoms start, end of story. But the body is rarely that simple.

Many women still experience hot flashes even when their hormone levels are being supported. That tells us something important. These symptoms are often coming from deeper layers of regulation, not just the ovaries.

Your brain and nervous system play a huge role in temperature control.

When your stress response is constantly activated, your internal thermostat becomes more reactive. That can show up as sudden heat surges, night sweats, and disrupted sleep. Over time, chronic stress and nervous system overload make it harder for the body to self-regulate smoothly.

Your liver and detox pathways matter here too. When hormone metabolism and toxin clearance slow down, those imbalances can amplify symptoms and make hot flashes more intense and persistent.
This is why a whole-body approach is so important. Hormones matter, but so do stress resilience, nervous system health, sleep, nutrition, detox capacity, and metabolic balance.

Your body isn’t malfunctioning. It’s communicating. And when we listen carefully, we can support it far more effectively than by chasing symptoms alone.

Blood Work You Should Check Before Starting a GLP-1

GLP-1 medications influence insulin signaling, appetite regulation, gastric emptying, and overall metabolic demand. Because of these systemic effects, baseline blood work is essential before starting therapy. Labs provide objective insight into glucose control, insulin resistance, liver and kidney function, thyroid status, inflammation, and nutrient stores. Without this data, important contributors to weight gain, fatigue, or metabolic dysfunction may go unrecognized.

Baseline testing also establishes a reference point for safety monitoring. Changes in kidney function, liver enzymes, electrolytes, or blood glucose can occur over time, and having pre-treatment values allows for appropriate interpretation. Monitoring helps distinguish medication-related changes from pre-existing conditions and ensures that treatment remains medically appropriate as weight and metabolic markers shift.

In addition, blood work can identify deficiencies that may worsen with appetite suppression, including iron, vitamin B12, vitamin D, and protein status. Addressing these proactively reduces the risk of fatigue, hair loss, muscle loss, and weakness during treatment. GLP-1 medications are most effective and safest when used within a comprehensive, data-driven care plan rather than in isolation.
