Heartland Community Health Center Comprehensive primary care, dental, and mental health for all, regardless of income or insurance status. Heartland accepts both insured and uninsured patients.

Uninsured patients pay on a sliding fee scale, which makes quality care affordable at Heartland. Heartland participates in the Increase the Reach program. This Increase the Reach grant program is supported by the Centers for Disease Control and Prevention (CDC) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $79,278,482 with 100 percent funded by the CDC/HHS. We heard your feedback, and we want it to be easier to connect with us. Starting Monday, Feb. 9, we will be implementing a new call system designed to help you:
✔️Spend less time waiting
✔️Get connected faster
✔️Communicate more easily with your care team

When you call, you will be greeted by a team member (not an automatic system) who will connect you directly to the right department.

If the team you need is not immediately available, we will explain your options and let you know what to expect, including voicemail or a callback when appropriate.

We are excited to announce that Jeff Hill, PMHNP, is joining our Psychiatry team.

Jeff is a Psychiatric Nurse Practitioner, bringing experience across outpatient psychiatry, emergency medicine, and primary care. At Heartland, Jeff will provide trauma-informed, evidence-based psychiatric care. He will work closely with family medicine and therapy teams to support patients through coordinated, whole-person care.

Multiple viruses are circulating locally right now, including Covid-19, influenza, RSV, and norovirus.

This is typical for this time of year, but it’s a good reminder to take extra precautions, especially for young children, older adults, or anyone with chronic health conditions.

To help reduce spread make sure you and your family:
• Stay up to date on vaccines
• Wash hands often with soap and water
• Cover coughs and sneezes
• Stay home when sick if possible

🔹Delayed start around 9:20am for routes 30-43.
🔹Routes 1-12 to run as scheduled along with paratransit.

Winter weather cancellations

âť„ Friday, January 23
🔹On Demand service is cancelled (8 p.m. onward).
🔹Routes 1-44 and paratransit are planned to run as scheduled. If weather conditions worsen, service updates will be communicated through our channels.

❄ Saturday, January 24 – All service is cancelled. No overnight On Demand service.

❄ Sunday, January 25 – On Demand service is cancelled.

❄ Monday, January 26 – Service planned to resume as usual. Service delays are possible, especially in the early morning. A late start may be necessary if temperatures drop below the current forecast. Extremely low temperatures can cause operational issues with our diesel and electric fleets.
