As the only fellowship-trained laryngologist in Utah north of Salt Lake City, Dr Meacham has special training and interest in disorders of voice and swallowing. As such, we have developed a voice center at Tanner Clinic with specialized equipment not found in most otolaryngology offices. High-definition stroboscopy allows exquisite detail of the vocal cords to be viewed with slow motion vibration. Transnasal esophagoscopy allows us to examine the esophagus in the office without the conventional need for IV sedation. Equipped to deal with all things "throat," we hope to be able to assist you in whatever way we can.

This MRI shows the resonance chamber of the pharynx and how dynamic it can be for a well-trained singer. For those that have problems with their singing voice and/or speaking voice, the single most common solution is to connect them with a voice therapist to relax the pharyngeal and laryngeal muscles.

Do you know someone that is always clearing their throat? It could be a vocal cord contact ulcer. These can be caused by any irritant on the voice box such as acid reflux, nasal secretions, or even throat clearing itself. Treatment can consist of acid reflux control, inhaled steroids, and voice therapy.

Trouble swallowing? It could be cricopharyngeal hypertrophy, which is an overly tight muscle at the top of the esophagus. Because this muscle will not relax, food has a hard time passing down to the stomach. Treatment consists of injection of botox into the tight muscle (see picture), and then if there is successful improvement in swallowing, a more permanent procedure can be done.

Ever wonder what your vocal cords look like when you get a case of laryngitis? See here...they get red and thickened and don't vibrate smoothly. To get better, it's best to rest your voice and drink plenty of fluids. If hoarseness lasts beyond a few weeks, you should be seen by your doctor.

If you ever have a sudden bout of complete hoarseness that comes after vocal cord trauma (yelling, singing, or even sneezing), it may be a vocal cord hemorrhage. Voice rest (no talking, singing, whispering) is important after this type of injury to prevent scarring of the vocal cord.

Hoarse voice and older than 60? It could be vocal cord atrophy, or volume loss. Just as the muscles in the arms and legs lose tone and volume with age, so also the muscles in the vocal cord can shrink as we get older. A simple in-office procedure can restore a less rough, more youthful voice.

Swallowing problem? It could be a Zenker's diverticulum.

After years of swallowing against an overly tight cricopharyngeus (CP) muscle in the upper esophagus, a pouch forms and collects food, which is sometimes regurgitated by the patient. Patients feel much better after a procedure to divide the CP muscle.

What is a vocal cord polyp? Polyps originate on the surface of the vocal cords usually after some type of trauma such as screaming, shouting, or singing inefficiently. Small polyps may resolve on their own with voice rest or voice therapy, but most usually need surgical removal.
