***CHARITY RAFFLE!! Visiting us for primary care or urgent care? Doodle a scratch-art leaf during your visit and we’ll display it with the others. For every leaf entered, the value of the raffled prize goes up $1 and we will donate $2 to local food banks in Lebanon & Sweet Home. Winning leaf will be chosen on November 25th and donations made by end of the month. One leaf per visit. If you bring your kiddo under 10 with you, they can make a leaf with or without an appointment!
