At Embassy of Lebanon, we provide compassionate short- and long-term care, memory care, and rehabilitation. We strive to give you all the comforts of home without the stress. Our 79-bed facility is conveniently located off Columbus Avenue, so our patients have access to the surrounding community businesses and activities. We offer private and semi-private rooms of various sizes to suit your expectations. Our care team is fully staffed with RNs, LPNs, and nursing assistants, and therapists. These staff members work 24/7 to develop and maintain treatment catered explicitly for you by working your medical history and needs into a comprehensive care plan.

Lebanon Health Care Center is a place you or your loved one can call home. From the beautifully landscaped grounds, to the carefully planned activities – this is a great community to join. We also operate an on-site, state-of-the-art post-acute skilled rehabilitation center.
